Many states had not even identified surveillance centres and in others they had not started functioning. In 14 states, including Andhra Pradesh, Assam, West Bengal, UP and Bihar, there had been no survey review of infant mortality and morbidity due to these vaccine preventable diseases. Further, in some states, even when polio was reported from the area under immunisation coverage, there had been no investigation.
